Inpatient Toxicology Consult Service Clinical Training

Hackensack University Medical Center is a quaternary referral center serving northern New Jersey and the New York metropolitan area. Fellows lead a busy inpatient toxicology consultation service managing:

​

  • Intentional overdoses and unintentional overdose

  • Chronic Toxicities 

  • Severe cardiotoxic and neurotoxic ingestions

  • Toxic alcohols and metabolic poisonings

  • Advanced hepatic and renal failure due to toxins

  • Transplant and advanced organ support cases

  • Complex ICU toxicology cases

  • Occupational and Industrial exposures and toxicities

  • Environmental exposures and envenomations

  • Drug intoxication and withdrawal syndromes

  • Medication-Assisted Treatment of Substance Use Disorders
     

Fellows gain substantial autonomy while working closely with experienced faculty toxicologists.

Critical Care & Advanced Therapies

​

As a high-acuity academic center, HUMC provides fellows experience with:

  • Hemodialysis and CRRT for toxicologic emergencies

  • ECMO in poisoning-induced cardiogenic shock

  • Advanced airway and resuscitative management

  • Whole bowel irrigation and enhanced elimination

  • Complex transplant-related toxicology

Fellows work closely with cardiothoracic ICU, transplant services, nephrology, and critical care teams

Outpatient Medical Toxicology Fellow Clinic

​

The Outpatient Medical Toxicology Clinic at Hackensack University Medical Center provides specialized evaluation and management of patients with complex toxicologic, medication-related, and exposure concerns.

Our board-certified medical toxicologists deliver expert, evidence-based care for patients with acute and chronic toxicologic conditions. We serve both adult and pediatric populations and accept referrals from primary care providers, specialists, emergency departments, and inpatient services.

  • Adverse drug reactions

  • Polypharmacy complications

  • Drug–drug interactions

  • Medication toxicity follow-up

  • Post-overdose recovery management

Substance Use & Withdrawal

  • Complications of substance use

  • Withdrawal syndromes

  • Harm reduction counseling

  • Coordination with addiction medicine services

Environmental & Occupational Exposures

  • Heavy metal exposure (lead, mercury, arsenic)

  • Mold and environmental toxin concerns

  • Chemical workplace exposures

  • Carbon monoxide exposure follow-up

Toxicologic Sequelae

  • Persistent neurologic symptoms after poisoning

  • Chronic symptoms following toxic exposures

  • Laboratory interpretation for abnormal toxicology results

  • Consultation regarding unclear exposure syndromes

Envenomations & Exotic Exposures

  • Snake, spider, and marine envenomation follow-up

  • Plant and mushroom toxicity evaluation

  • Travel-related toxic exposures

Research & Scholarship

We emphasize scholarly productivity and academic development. Hackensack University Medical Center has a strong research infrastructure within the network, hospital, and the department of Emergency Medicine. 

Fellows are supported to:

  • Present at the annual meeting of the North American Congress of Clinical Toxicology and the American College of Medical Toxicology Scientific Meeting

  • Publish in peer-reviewed journals

  • Participate in multicenter toxicology studies

  • Develop case reports and translational research

  • Engage in Quality Improvement Initiatives and toxicovigilance projects

  • Collaboration with the New Jersey Regional Poison Control Center

Areas of scholarly focus may include:

  • Novel treatments for severe poisoning

  • Toxicologic critical care

  • Substance use epidemiology

  • Transplant-related toxicology

  • Pediatric exposures

  • Regional toxicologic trends in the New York–New Jersey corridor
     

Protected academic time and structured mentorship are core components of the fellowship.

Curriculum

Our longitudinal curriculum includes:

  • Weekly toxicology didactics

  • Case based conferences

  • Journal club

  • Board review sessions

  • Forensic toxicology exposure

  • Occupational Toxicology exposure

  • Addiction medicine interface

  • Public health and regulatory toxicology

Fellows also participate in medical student and resident education.

Location

Located in Hackensack, NJ, our institution serves a diverse patient population and functions as a regional referral center for complex toxicologic cases across the Hackensack Meridian Health system and surrounding hospitals. Fellows also benefit from a vibrant community with abundant cultural, outdoor, and academic opportunities. Fellows may also choose to live in New York City, with an easy 20-minute commute by car and convenient public transportation options.
